What is Kidney Cancer?

Kidney cancer — most commonly Renal Cell Carcinoma (RCC) — arises from the lining of the renal tubules. Clear cell RCC is the most frequent subtype (~75%) and is highly vascular, making anti-angiogenic (anti-VEGF) therapy particularly effective. Other subtypes include papillary, chromophobe, and collecting duct carcinoma. RCC is often discovered incidentally on imaging. The disease is notable for its response to immunotherapy and targeted agents, with combination immunotherapy-TKI regimens now producing remarkable outcomes in advanced disease.

Subtypes: Clear cell (75%), papillary, chromophobe, collecting duct RCC
Key pathways: VHL/HIF/VEGF pathway drives clear cell RCC — target for TKIs
First-line: Pembrolizumab + axitinib or nivolumab + cabozantinib now preferred over sunitinib alone
Hereditary: VHL syndrome, hereditary papillary RCC — genetic testing for high-risk families

Symptoms & Early Signs

Early recognition of symptoms leads to timely diagnosis and significantly better treatment outcomes. Do not ignore these warning signs.

🩸

Blood in Urine

Haematuria — visible or microscopic blood in urine — is the classic presenting symptom of kidney cancer.

😣

Flank / Back Pain

Dull persistent pain in the flank or back on the affected side — often confused with musculoskeletal pain.

🫘

Palpable Abdominal Mass

A firm, non-tender lump felt in the flank or upper abdomen in large kidney tumours.

🌡️

Paraneoplastic Fever

Unexplained persistent fever not due to infection — a paraneoplastic feature of renal cell carcinoma.

📉

Weight Loss & Fatigue

Significant unexplained weight loss and anaemia-related fatigue are systemic symptoms of advanced RCC.

🔴

Hypertension

High blood pressure caused by tumour compression of renal vasculature or renin hypersecretion.

Causes & Risk Factors

Understanding risk factors helps in early detection and prevention. Consult Dr. Gupta if you are at elevated risk.

01
Smoking

Doubles kidney cancer risk — dose dependent relationship with duration and amount smoked.

02
Obesity

Excess body weight is one of the strongest modifiable risk factors for RCC, particularly in women.

03
Hypertension

Chronic high blood pressure and antihypertensive medications (especially diuretics) are associated risk factors.

04
VHL Syndrome

Von Hippel-Lindau hereditary syndrome causes multiple bilateral clear cell RCCs at a younger age.

05
Dialysis & CKD

Long-term dialysis and acquired cystic kidney disease in patients with chronic kidney disease raise RCC risk.

06
Occupational Exposure

Exposure to trichloroethylene, cadmium, and asbestos in industrial settings increases kidney cancer risk.

Risk factors

Incidental Discovery — Routine Ultrasound Saves Lives

Many kidney cancers are discovered incidentally during abdominal ultrasound or CT done for other reasons. At this early stage, surgical cure is highly achievable. Dr. Gupta recommends imaging in anyone with persistent haematuria or unexplained flank pain.
